The report categorizes influences into health services, individual behavior, physical determinants, and social determinants. Individual behavior accounts for the largest share of factors impacting both high utilization and admission/readmission avoidance, representing over 50% in both categories. Social determinants such as food services, transportation, and financial assistance make up approximately 9%, while health services account for around 30-36%. The analysis underscores the importance of addressing individual behavior and social factors to improve patient outcomes and reduce healthcare utilization.
